We will need the following paperwork in order to
get each swimmer properly registered with the Duke Aquatics swim
team:
Page 3 completed and signed.
This is an
annual membership needed for insurance purposes. Swimmers
will receive 6 bi-monthly issues of Splash
Magazine.
(Please do
not make a separate check out to USA Swimming—this fee is
included in the registration fee as shown in number 4 below.)
3) Copy
of Birth Certificate
For swimmers
new to year-round swimming, this is necessary for USA Swimming to
verify each swimmer’s correct name and age. Swimmers who have been previously registered
with a different year-round team need to complete a NC
Transfer form.
4) $100
Registration Fee ($50 for Pre-Competitive group) + First
Month’s Dues
Your annual
USA Swimming registration fee is included in this fee; includes
team cap & t-shirt (except for Pre-Comp)
